垂体腺瘤手术治疗效果分析

目的 分析垂体腺瘤合理有效的手术治疗方式.方法 对194例垂体瘤采用经鼻-蝶入路、经颅入路手术治疗的疗效进行分析.结果 经鼻-蝶组和经颅手术组肿瘤的次全切除率以上分别为95%和91%,术后放射治疗后肿瘤完全消失可达到90%.结论 鼻-蝶入路手术治疗可作为各类垂体微小腺瘤和部分大型腺瘤首选的治疗手段;采用经颅入路显微手术和术后辅助放射外科治疗是提高垂体巨型腺瘤全切除率、降低复发率的重要手段.

Study on Different Surgical Operation Methods in Treatment of Pituitary Adenoma

Objective To explore the reasonable and effective therapeutical methods in the treatment of pituitary adenoma. Methods Total 194 cases of pituitary adenoma treated by different methods was studied retrospectively. Therapeutical effects and its complications were also analyzed. Resuits The rate of tumor subtotal removal were 95% and 91% respectively in transsphynoidal group and transeranial group. The 90% of adenoma operated completely disappeared with radiation. Conclusion The pituitary tumor could be treated by transsphenoidal and transsubforehead approach of surgical operation. Transsubforehead approach with microsurgical technique and radiation can improve the rate of tumor total removal in the treatment of giant pituitary adenoma and decrease the reeurrence rate.
