慢性乙型重型肝炎的抗病毒治疗

目的分析慢性乙型重型肝炎抗病毒治疗的体会,以提高其抢救成功率。方法运用流行病学调研方法 ,分析2007年传染病专科医院147例慢性乙型重型肝炎患者的救治结果 ,并报告抗病毒治疗对提高生存率的影响。结果慢性乙型重型肝炎内科治疗的病死率约42.86%（63/147）,较核苷（酸）类抗病毒药物应用前显著下降;符合纳入标准、入院超过2周的115例患者中使用核苷（酸）类抗病毒治疗者达87.83%（101/115）,病死率为26.96%（31/115）。本组病例既往抗病毒治疗过程中发生慢性重型肝炎者25例,占21.74%（25/115）。结论抗病毒治疗可显著降低慢性乙型重型肝炎患者的病死率;需对抗病毒治疗过程中的慢性乙型肝炎患者加强长期监护,杜绝发生与抗病毒治疗相关的重型肝炎。

Analysis on antivirus therapy in severe chronic hepatitis B patients

Objective To observe the clinical effects of antiviral therapy on severe chronic hepatitis B.Methods All 147 cases with severe chronic hepatitis B were included in this study,who were hospitalized patients in Beijing Ditan Hospital in 2007.Outcomes and fatality rate were documented.Results The mortality rate was 42.86%（63/147）.Total of 115 cases stayed in hospital for more than two weeks and the mortality rate was 26.96%（31/115）.About 87.83%（101/115）patients received antiviral therapy during antiviral therapy,21.74% patients（25/115）developed to severe chronic hepatitis.Conclusions Antiviral therapy can remarkably decrease the mortality rate of patients with severe chronic hepatitis B.More attention should be paid to the patients with chronic hepatitis B who received antiviral therapy in order to reduce the incidence of severe chronic hepatitis.
