超声诊断喉癌的准确性及诊断医师之间的一致性研究

目的研究超声诊断喉癌的准确性及诊断医师之间的一致性。方法对121例临床疑似喉癌的病例进行超声检查并采集图像,再分别由3位超声医师根据采集到的图像和喉癌超声诊断标准分级诊断为Ⅰ～Ⅵ级。对照病理结果后构建ROC曲线,计算曲线下面积(Az)以评价诊断喉癌的准确性。分别计算κ值以评价3位医师之间的一致性。结果 118例患者获得了满意的超声图像。3位医师对应的Az值分别为0.939、0.954和0.947,各医师之间的κ值分别为0.75(P0.001)、0.71(P0.001)、和0.86(P0.001)。结论超声诊断喉癌的准确性和一致性较高,在喉癌的影像诊断方面有着良好的应用前景。

Accuracy and Interobserver Agreement in Diagnosing Laryngeal Cancer with Ultrasonography

Objective To investigate the accuracy and interobserver agreement in diagnosing laryngeal cancer with ultrasonography.Methods One hundred and twenty-one cases suspected laryngeal cancer were detected with ultrasonography and their images were reserved.Three ultrasonographers individually gave confident rating reports as Ⅰ～Ⅵ grades based on the reserved images and the ultrasonography standard in diagnosing laryngeal can-cer.Compared the reports with pathologic results,receiver operating characteristic (ROC) curves were constructed,and the areas under the curves (Az) were calculated to test the accuracy.The κ values were calculated to test interobserver agreement.Results Good images were obtained in 118 cases.The Az values of 3 ultrasonographers were 0.939,0.954 and 0.947,respectively.The κ values between 3 observers were 0.75 (P<0.001),0.71 (P<0.001) and 0.86 (P<0.001),respectively.Conclusions Ultrasonography derives considerable accuracy and interobserver agreement in diagnosing laryngeal cancer.It is a promising imaging device on the diagnosis of laryngeal cancer.
