| 摘 要： | 目的探讨结肠成形术在低位直肠癌保肛手术中的应用价值。方法将低位直肠癌患者随机分为两组，观察组34例（结肠成形术低位直肠或肛管吻合）和对照组42例（结肠与远端直肠或肛管直接吻合），观察比较两组患者术后肛门控便功能恢复情况及并发症发生率。结果肛门功能观察组大便控制明显优于对照组，术后6个月观察组平均3次／d，对照组平均8．5次／d；术后12个月观察组平均1．5次／d，对照组平均5次／d，两组差异有统计学意义P〈0．01。结论结肠成形术后肛门功能恢复明显优于直接吻合组。 |

| Abstract: | Objective To observe the clinical value of sigmoid coloplasty for lower rectal cancer surgery. Methods Thirty-four patients underwent sigmoid coloplasty （treatment group） while 42 patients received straight sigmoidorectostomy （control group）. Anal function and complication rate were assessed and compared between the two groups. Results Average stool frequency were 3 times a day in the treatment group and 8.5times a day in the control group after 6 months. Average stool frequency werel. 5 times a day in the treatment group and 5 times a day in the control group after 12 months. There was significant difference between the two groups 12 month after operation （ P〈0.01）. Conclusion Compared with straight sigmoidorectostomy, coloplasty pouch applied in lower rectal cancer surgery can significantly improve the anal function without increasing complication rate. |
