Abstract:Objective:To explore the application value of dual-energy CTA in predicting hematoma growth.Methods Clinical and imaging data of 167 patients were collected,including 57 patients in the hematoma growth group and 110 patients in the group without hematoma growth.The clinical and imaging data that differed between the two groups were enrolled in the logistic regression analysis using the Conditional Forward Stepwise,and the area under the ROC curve was calculated to compare their advantages in predicting hematoma enlargement.Results The time from onset to CT scan,initial blood volume,"blend sign","spot sign"in dual-energy and"spot sign"in conventional CT in the hematoma growth group were(3.25±1.20)hours,(33.83±12.55)ml,25 cases,36 cases and 32 cases,respectively,and(3.94±1.38)hours,(23.19±12.88)ml,13 cases,6 cases and 13 cases in the group without hematoma growth.They were significantly different between the two groups.The sensitivity of"blend sign",dual-energy"spot sign"and conventional"spot sign"in predicting hematoma enlargement were 43.9%,63.2%,56.1%,and the specificity was 88.2%,94.5%,88.2%,respectively.The area under the characteristic curve was 0.66,0.79 and 0.72,respectively.Final the initial bleeding volume(OR:1.07),time from onset to CT scan(OR:0.63),and"point sign"in dual-energy(OR:33.42)were entered into the Logistics Regression Analysis,and the area under the ROC curve for combined parameters to predict hematoma growth was 0.89.Conclusion Dual-energy CTA has higher value in predicting intracranial hematoma enlargement than conventional CTA scans,and the combination of clinical and imaging findings can further improve the predictive accuracy.
