超声心动图评价系统性红斑狼疮患者治疗前后左心功能的变化

目的：应用超声心动图（UCG）检测系统性红斑狼疮（SLE）患者治疗前后左心功能的变化，为SLE的临床监测、病情评价提供重要参考。方法：用M型超声心动图获取左室功能指数：心搏量（SV）、心输出量（CO）、射血分数（EF）。心尖四腔切面获取二尖瓣血流，测量舒张功能参数：舒张早期E峰最大流速（Ve）、舒张晚期A峰最大流速（Va）及Va／Ve。心尖五腔切面获取主动脉瓣血流，测量收缩功能参数：射血前期（PEP）、左室射血期（LVET）及PEP／LVET（P／L）。结果：SLE组治疗前后Va／Ve均显著高于对照组（P〈0．01），治疗前Ve降低（P〈0．05），Va升高（P〈0．05）；治疗后Ve与对照组比较差异无显著性（P〉0．05），Va升高（P〈0．05）。治疗前Va／Ve高于治疗后（P〈0．05），治疗前后Ve比较差异有显著性（P〈0．05），Va比较差异无显著性（P〉0．05）。SLE组治疗前后P／L均高于对照组，治疗前CO也显著高于对照组（P〈0．01），但治疗后与对照组比较差异无显著性（P〉0．05），SV，EF治疗前后与对照组比较差异均无显著性。治疗前CO高于治疗后（P〈0．05），但P／L，SV，EF治疗前后比较差异无显著性（P〉0．05）。结论：UCG作为一项无创、敏感、价廉的检测手段，可早期定量显示SLE患者心功能的变化，使大量狼疮性心肌受累患者得以检出，为SLE的临床监测、病情评价提供重要参考。

Evaluating changes of left ventricular function using echocardiography in patients with systemic lupus erythematosus before and after treatment

Objective：To discuss the change of LV function usingUCG in patients with SLE before and after treatment,in order to prove essential references for clinical monitoring,condition assessment. Methods：LV function index ：SV, CO, EF were obtained using M-mode UCG,diastolic function index ： Ve, Va, Va/Ve were measured in mitral flow of Apical four chamber view ;systolic function index：PEP,LVET,P/L were measured in aortic blood flow of Apical five chamber view. Results ：The results of this study showed that Va/Ve in the patients with SLE before and after treatment was significantly higher than that in control group（P〈0.01）. Va was higher. Ve was lower before treatment （ P 〈 0.0 5 ）. Va was still higher （ P 〈 0. 0 5 ）. Ve was not significantly different after treatment （ P 〈 0. 0 5 ）. Va/Ve was higher before treatment than that after treatment （P〈0. 05）. Ve was significantly different before and af- ter treatment （P〈0.05）. Va was not significantly differet before and after treatment （P〉0. 05）. P/L in the patients with SLE Before and after treatment was higher than that in control group（P〈0.01 ,P〈0. 05）. CO was higher than that in control group before treatment （P〈0.05）, but was not significantly different after treatment （P〈0.05）. SV, EF was not significantly different before and after treatment than that in control group（P〈0.05）. P/L,CO was higher before treatment than that after treatment （P〈0. 05）. SV,EF was not significantly different before treatment than that after treatment. Conclusion ：As a non-invasive ,sensitive and inexpensive detection means ,UCG can show quantitative changes of LV function in SLE patients,so it can provide important references for clinical monitoring,condition assessment of SLE patients.
