硅油治疗复杂性视网膜脱离疗效分析

目的 分析玻璃体切割结合注射硅油顶压治疗复杂性视网膜脱离的疗效和并发症。方法 对我院近年来开展的9例复杂性视网膜脱离应用硅油顶压治疗的患者随访10～20个月，平均13个月。其中硅油注入保留在眼内的时间3～20个月，因硅油乳化而取出。结果 硅油术后患者视力均有不同程度提高，视网膜复位，术后并发症主要有：并发性白内障、继发性青光眼、硅油乳化和角膜变性；而取出硅油后并发症主要有：视网膜脱离复发、眼内硅油残留和继发性青光眼。结论 玻璃体切割结合注射硅油治疗复杂性视网膜脱离，提高了手术的治愈率。注油后或取油后的并发症经适当处理，仍可取得满意效果。

Complications and treatment of cases complicated retinal detachment by with silicone oil injection

Objective To analyze the efficacy and complications of vitreoectomy with silicone oil injection for complicated retinal detachment. Methods Nine cases of complicated retinal detachment received silicone oil therapy and were followed up 10 to 20 months (average 12 months). The longest case following-up with silicone oil retention in the eye was 20 months,while the shortest with silicone oil taken out was 3 months after surgery because of emulsification. Results The visual acuity and retinal reattachment of all patients were improved in different degrees. The main post operative complications were complicated cataract, secondary glaucoma, silicone oil emulsification and cornea degeneration. The main complications after oil taken-out were retinal detachment, oil residuel in the eye, secondary glaucoma. Conclusion Vitreoectomy with silicone oil injection is effective for complicated retinal detachment. Complications can be minimized with suitable treatment.
