颈上部血管损伤急诊救治13例临床分析

目的：总结颈上部血管损伤急诊救治的经验与效果。方法：回顾1992年8月-2006年3月间13例颈上部血管损伤患者的临床资料，对血管损伤部位、处理方法等进行分析。结果：除1例刀刺伤致颈内动脉破裂外，其余12例均合并其他组织损伤。颈内动脉损伤2例。颈外动脉及其分支损伤10例，颈内静脉损伤1例。13例患者经多学科综合治疗，痊愈12例，并发脑栓塞死亡1例，治愈率93．7％。结论：救治应以受伤原因与伤情为依据。早期诊断、及早手术探查、正确的抗休克等，是救治成功的关键。

Emergency management of superior cervical blood vessel injuries in 13 patients

PURPOSE: To review and summarize the experience of emergency management of superior cervical blood vessel injuries in 13 consecutive patients. METHODS: The clinical data of 13 patients with superior cervical blood vessel injuries treated between August 1992 and March 2006 were reviewed. The sites of vascular injuries and the treatment methods were analyzed. RESULTS: 12 of 13 patients had composite injuries. 2 patients had injury of the internal carotid artery, 10 patients had injury of the external carotid artery and its branches, 1 patient had injury of the internal jugular vein. After multidisciplinary management, 12 patients were cured without major complications, while one patient died due to cerebral embolization. CONCLUSIONS: Emergency management for these patients with cervical vascular injuries should be based on the causes of injury and the patients' performance status. Early diagnosis, timely operation and appropriate anti-shock therapy are the key factors to successful treatment.
