胫腓骨骨折合并软组织损伤治疗临床分析

目的比较外固定支架,钢板内固定以及非扩髓髓内钉方式对治疗胫腓骨骨折合并软组织损伤的疗效。方法回顾性分析我院2004年4月至2011年5月收治的124例胫腓骨骨折合并软组织损伤的患者,其中采用单臂外固定支架治疗的48例,钢板内固定治疗的33例,非扩髓髓内钉的43例。对3组患者的一般资料、软组织分型、骨折术后平均愈合时间、骨折感染率、骨折延迟愈合及不连率进行统计学分析。结果非扩髓髓内钉组在术后平均愈合时间、骨折延迟愈合及不连率方面明显优于钢板内固定组和单臂外固定支架组（P〈0.05）;3组患者在术后感染发生率的比较中无统计学差异（P〉0.05）。结论采用非扩髓髓内钉法治疗胫骨骨折合并软组织损伤,其疗效确切,并具有术后愈合时间短、畸形愈合少等优点,值得在临床推广应用。

Clinical Analysis on the Treatment of Tibiofibula Fractures with Injury of Soft Tissue

Objective To compare the results of the treatment of the mono-lateral external fixation,dynamic compression plate fixation and the unreamed intramedullary nailing for the tibial and fibula fractures with injury of soft tissue.Methods 124 cases in our hospital between April 2004 to May 2010 were included in our investigation.We have compared the clinical data,the type of soft tissue,the average healing time,the rate of infection,the delayed union and rate of nonunion fracture between the three groups.Results The group of unreamed intramedullary nailing performed the best among the 3 fixations in the average healing time,the rate of delay union and the nonunion fracture.The difference between the 3 groups was not significant in the rate of infection.Conclusion For the tibiofibula fractures with injury of soft tissue,the unreamed intramedullary nailing is more superior than mono-lateral external fixation and dynamic compression plate fixation.
