十二指肠营养管在食管贲门癌治疗中的应用

目的：研究食管贲门癌切除术中放置十二指肠营养管进行早期肠内营养在维持机体营养,促进恢复及在防治术中后并发症与吻合口瘘的作用。方法：将574例食管贲门癌病人随机分为A,B两组,A组术后常规静脉输液,B组术中放置十二指肠营养管,术后3d给予肠内营养,两组病人监测术前1d,术后5d和术后10d的体重,血浆蛋白及尿中24h尿素氮,结果：两组病人在体重,血浆蛋白及尿素氮方面存在明显差异（P＜0．05）,在术后并发症方面差异明显（P＜0．01）,结论：十二指肠营养管进行肠内营养具有易于维持机体营养,促进机体康复,尽快恢复正氮平衡,减少术后并发症的优点。

THE APPLICATION OF DUODENAL FEEDING TUBE IN THE TREATMENT OF ESOPHAGEAL OR CARDIAC CANCER

Objective:To study the effect of duodenal feeding tube for the patients with esophageal or cardiac cancer on maintaining body nutrition,promoting recovery,preventing and treating postoperative complication.Methods:Clinical trial was performed on 574 patients with esophageal or cardiac cancer.The patients were divided into Group A(283 cases) and Group B(291cases ).Group A was given routin intravenous infusion after operation.Group B was placed duodenal feeding tube and given enteral nutrition three days after operation.Two Groups were determined in body weight,plasma protein and 24h urine urea nitrogen(UUN)at one day before operation five days and ten days after operation respectively.Results:There were significant differences between the two groups in body weight,plasma protein,UUN,and postoperative complication(P<0.05).Conclusion:The advantage of duodenal feeding tube is easily to maintain body nutrition,to promot recovery,to quickly recover positive balance of nitrogen and to reduce postoperative complication.
