ART治疗反复IUI失败病人的实验室结果分析

目的评价反复宫腔内人工授精(IUI)失败的患者继续应用体外授精(IVF)方式治疗的实验室结果。方法对2010年1—12月在本中心应用辅助生殖技术(ART)治疗反复IUI失败病人进行回顾性分析。结果共有92对IUI患者转入ART治疗,其中86个周期进行IVF治疗(对照组),共17周期进行补救卵细胞浆内单精子注射(R-ICSI)治疗,有6周期进行ICSI治疗。与同年其他原因进行IVF治疗的周期相比,86个周期反复IUI失败组的卵母细胞成熟率以及受精率明显低于对照组,差异有统计学意义;2PN率,卵裂率,D3优质胚胎率均比对照组低,但差异无统计学意义。反复IUI治疗失败的患者采用R-ICSI与其他R-ICSI相比,卵子的成熟率,受精率,2PN率,卵裂率以及优质胚胎率差异均无统计学意义。结论反复IUI治疗失败的患者中存在着受精失败的可能,采取短时授精联合R-ICSI可以有效的解决受精障碍和低受精的情况。

Analysis on outcomes of patients who failed in repeated IUI and went on with assisted reproductive technique

Objective To evaluate the results of patients who failed in repeated intrauterine insemination(IUI) and went on to undergo assisted reproductive technique(ART).Methods Retrospective analysis was conducted on the patients who failed in repeated IUI and were treated by ART from January to December 2010.Results Of the 92 pairs of partners who totally changed over to ART,86 received in vitro fertilization(IVF) and 17 of them received rescue intracytoplasmic sperm injection(R-ICSI) for six cycles after IVF.To compare with those who received IVF treatment due to other reasons in the same year,the mature rate and fertility rate of the oocytes were statistically lower in the 86 ones.Conclusion There is a possibility of fertilization failure in the patients who failed in repeated IUI treatment.Fertilization obstacles and low fertilization can be solved availably by receiving short-term fertilization plus R-ICSI.
