粘连性肠梗阻的临床治疗效果研究

目的观察肠粘连导管在粘连性肠梗阻中临床疗效。方法以入住我院的粘连性肠梗阻患者为研究对象,随机的将患者分为两组,A组应用常规胃管进行胃肠减压,在无效时采用肠梗阻导管进行治疗,B组应用透视下插入法应用肠梗阻导管进行胃肠减压,观察治疗前后的各项指标变化。结果实用肠梗阻导管后,发现B组腹痛腹胀缓解率高于A组,差异有统计学意义（P〈0.05）;自主排气时间B组优于A组,差异有统计学意义（P〈0.05）;第一个24 h消化液引流量比较,B组优于A组,差异有显著性意义（P〈0.05）;X线片平片观察,A组常规胃肠减压,腹平片未见明显变化,使用肠梗阻导管,可见腹片液平面减少或者消失,平均消失时间为（7.9±5.3）d;引流液性质：肠导管深黄色粪便样液体,伴臭味、黄色肠液而普通胃管为黄色肠液、绿色胆汁样液体,白色胃液。结论肠梗阻导管能有效治疗粘连性肠梗阻,改善患者症状,降低手术率,疗效优于普通胃管。

Study on the Clinical Effect of the Treatment of Adhesive Intestinal Obstruction

Objective To observe the clinical effect of intestinal adhesion catheter in adhesive intestinal obstruction.Methods as the research object,in our hospital patients with adhesive intestinal obstruction were randomly divided into two groups,group A were treated with conventional tube for gastrointestinal decompression,the invalid by intestinal obstruction catheter for treatment,group B were treated under the perspective application of insertion method application of ileus tube for gastrointestinal decompression,the changes before and after the treatment were observed.Results After using intestinal obstruction catheter,abdominal pain remission rate in group B was higher than that in group A,the difference was significant（P＆lt;0.05）;independent exhaust time in group B was better than in group A,the difference was significant（P＆lt;0.05）;the first 24 hours digestion drainage and comparison,B was superior to group A,the difference was significant（P＆lt;0.05）;X-ray plain film,group A in conventional gastrointestinal decompression,obvious changes of no use plain film of the abdomen,intestinal obstruction catheter,visible sternite liquid level was reduced or disappeared,the average disappeared time was 7.9±5.3 days;drainage properties： intestinal duct was deep yellow stool sample liquid,with the smell the yellow juice,and ordinary tube was yellow intestinal juice,green bile-like liquid,white juice.Conclusion intestinal obstruction catheter is effective in the treatment of adhesive intestinal obstruction,It can improve symptoms,reduce the operation rate,the curative effect is better than that of ordinary tube.
