益气养阴解毒法对非小细胞肺癌患者生活质量和免疫功能的影响

目的观察益气养阴解毒法对Ⅲ、Ⅳ期非小细胞肺癌（NSCLC）患者生活质量（QOL）改善作用及对免疫功能调节作用。方法采用随机、前瞻、对照研究方法，将61例证属气阴两虚的Ⅲ、Ⅳ期NSCLC患者随机分为中药联合化疗组（治疗组）29例、单纯化疗组（对照组）32例，治疗组用益气养阴解毒法结合化疗，对照组单用化疗，2组化疗均用NP、GP、TP方案。2周期后进行疗效评价。结果治疗组治疗后（采用美国肺癌治疗功能评价表FACT-L4．0）社会／家庭状况、功能状况积分较治疗前升高且有统计学意义（P〈0．05）；对照组治疗后QOL总积分、生理状况、情感状况积分较治疗前降低且有统计学意义（P〈0．05，P〈0．01）；2组治疗后比较，治疗组QOL总积分及各领域积分均高于对照组，其中功能状况领域积分2组比较有统计学意义（P〈0．05）。结论益气养阴解毒法能改善Ⅲ、Ⅳ期NSCLC患者生活质量，通过提高CD4/CD8比值、血清IL-2水平、降低血清sIL-2R水平调节免疫功能。

Effect of Yiqi Yangyin Jiedu Method on the Life Quality and Immune Function of Non-small Cell Lung Cancer Patients

Objective To study the effect of Yiqi Yangyin Jiedu Method on improving the quality of life （QOL） and regulating immune function in patients with stage III, IV non-small-cell lung cancer （NSCLC）. Methods We performed a randomized, prospective, controlled study in 61 NSCLC patients with deficiency of both Qi and Yin syndrome. The patients were randomized over Traditional Chinese Medicine （TCM） plus chemotherapy （TCM treatment group, n = 29） or chemotherapy alone （control group, n = 32）. TCM treatment was based on the ＂Benefiting Qi Nourishing Yin and Removing Toxicity Principle＂ combined with chemotherapy was used in the TCM treatment group. All patients received Define NP GP and TP. The efficacy of treatment was evaluated after two chemotherapy cycles. Results QOL features： The total QOL score （FACT-L 4. 0） and the following QOL features increased significantly before and after treatment in the TCM treatment group： in the field of society/family condition and function condition （ P 〈 0.05）. In the control group, in the field of the total QOL score, physical condition and affection condition decreased after treatment （ P 〈 0.05, P 〈 0.01）. After treatment, in the TCM treatment group, the total QOL score and each of the single constituting features were higher than that in the control group, especially in the feature ＂condition＂ （ P 〈 0.05）. Senun NK level in the control group after treatment was lower. A trend for higher serum IL-2 was observed in the treatment group compared with that in control group after treatment. Conclusions For patients with stage III, IV NSCLC and Qi and Yin deficiency, TCM treatment according to the benefiting Qi Nourishing Yin and Removing Toxicity Principle can improve QOL and has a trend to enhance CD4/CDs ratio and serum IL-2 level, decrease serum sIL-2R level.
