葡萄球菌对克林霉素的诱导性耐药研究和分析

摘    要:目的了解葡萄球菌对红霉素和克林霉素的耐药情况及对克林霉素诱导性耐药的发生率。方法K—B法检测葡萄球菌对红霉素和克林霉素的耐药性,头孢西丁纸片扩散法检测耐甲氧西林葡萄球菌,D试验检测克林霉素诱导型耐药。结果在152株红霉素耐药、克林霉素敏感的葡萄球菌中,85株为诱导型耐药,检出率为55.92%(85/152),其在耐甲氧西林金黄色葡萄球菌、甲氧西林敏感凝固酶阳性金黄色葡萄球菌、耐甲氧西林凝固酶阴性葡萄球菌和甲氧西林敏感凝固酶阴性葡萄球菌中所占比例分别为57.14%(8/14)、76.47%(26/34)、50.70%(36/71)和45.45%(15/33)。结论葡萄球菌诱导克林霉素耐药的发生率处于相对较高水平,临床细菌室应重视D试验以指导临床合理选择抗生素。

Abstract:Objective To investigate the drug resistance of Staphylococci to erythromycin and clindamycin and the prevalence of inducible clindarnycin resistance of erythrornycin-resistant Staphylococci in our hospital. Methods Kirby-Bauer method was performed to detect the resistance of erytbrornycin and clindarnycin against Staphylococci, and eefoxitin disk diffusion method was applied to detecting methicillin-resistant staphylococci (MRS), and D-test was used to detect inducible clindarnycin resistance of erythrornycin-resistant Staphylococci. Results Among 152 strains of erythrornyein-resistant and clindarnycin-sensitive Staphylococci, 85 strains (55.92% ,) were D test positive, and their constituent ratio in inducible clindarnyein-resistance of rnethieillin resistant staphylococcus aureus (MRSA), rnethieillin sensitive staphylococcus aureus (MSSA), rnethicillin sensitive coagulase-positive staphylococci (MRCNS) and rnethieillin sensitivecoagulase-positive staphylococci (MSCNS) was 57. 14% (8/14),76.47% (26/ 34), 50.70% (36/71) and 45.45% (15/33) respectively. Conclusion The prevalence of inducible elindarnycin resistance of Staphylococci is relatively high. D-test should be emphasized by clinical bacteriological laboratory so as to guide the rational use of antibiotics in the clinic.
