484例青年人肺癌临床分析

目的为研究青年人肺癌的临床特点和生存情况。方法回顾性分析484例年龄≤45岁的青年人肺癌患者资料,和同期3 267例非青年人肺癌比较临床特点构成和生存。采用SPSS 10.0软件分析青年人肺癌和非青年人肺癌临床特点和生存的差异。结果 484例青年人肺癌中男性有363例（75.0%）,女性121例（25.0%）。有吸烟史282例（58.3%）,重度吸烟者（≥400年支数）215例（44.4%）。和非青年人肺癌比较,青年人肺癌在性别、吸烟情况、合并慢性支气管炎、病理类型和分期等方面有明显差异,表现为女性、腺癌、小细胞癌及进展期（Ⅲ~Ⅳ期）等因素在青年人肺癌病例中所占比例较非青年人肺癌组高。青年人肺癌和非青年人肺癌在治疗情况和生存情况方面无明显差异。结论青年人肺癌和非青年人肺癌比较在多个方面有其特点,应该开展适合青年人肺癌特点的治疗和研究。

Clinical analysis on 484 young patients with pulmonary carcinoma

Objective To analyze the clinical characters of young patients with lung cancer.Methods The cancer registry files of the Zhejiang Cancer Hospital from 1992 to 2001 was reviewed and 3 751 patients with a documented diagnosis of lung cancer was identified.Among these patients,484（12.9%） were aged 45 or less.Patients′ clinical characters（age,gender,smoking status,family history of cancer,stage and histology） and survival were analyzed by statistical software SPSS version 10 and compared with those of elderly patients.Results Among 484 patients,363（75.0%） were men and 121（25.0%） were women.The smoking history was available in 282（58.3%） patients,among whom 215 were smokers and had a ≥20 pack-year history of smoking.The differences in the proportion of gender,stage,smoking status and histology,between the two age groups were statistically significant.Young lung cancer patients presented the advanced disease,adenocarcinoma,small cell cancer and female gender.The two groups had no difference in treatment and survival.Conclusion Young lung cancer patients have unique clinical characters,and special study is needed.
