新型隐球菌性脑膜炎18例临床分析

目的提高对新型隐球菌性脑膜炎的临床认识,减少误诊。方法回顾性分析了我院收治的18例患者的临床特点、确诊经过、误诊原因及防范措施。结果18例患者临床表现以发热、头痛、脑膜刺激征最多见。脑脊液常规检查以颅内压、白细胞、蛋白质增高,葡萄糖、氯化物降低最多见。病原体检查经反复脑脊液涂片墨汁染色及培养大多数能找到隐球菌。结论对临床表现脑脊液常规检查不典型的脑膜炎患者应常规做脑脊液涂片墨汁染色及培养找隐球菌,以尽早确诊。

18 Clincal analysis new type cryptococous meningitis

Objective: To raise the clinical knowledge of new type cryptococous meningitis and to reduce the error of diagnosis.Methods: To review the analysis of the clinical characteristic,the course of clear-cut diagnosis,the cause of mistake during diagnosis and measures for protection of the 18 patients came for medical advice.Results: Most of the clinical display of the 18 patients were favour,headache sign of meninx after stimulus.most of the result of the convention inspect of the cerebrospinal fluid were the risement of the pressure of the cranial cavity.the white blood cell.the protein and the depression of the glucose.the chloride,after repeatedly cerebrospinal fluid smear coloured by prepared Chinese ink and culture most of the patient can be found cryptococous in it.Conclusion: Looking for cryptococous by cerebrospinal fluid smear coloured by prepared Chinese ink and culture can definite diagnosis to the meningitis patients that aren't typical on clinical sign and routine cerebrospinal fluid.
