血管闭塞性脉管炎的彩色多谱勒超声诊断价值

摘    要:
目的探讨彩色多谱勒超声影像对血管闭塞性脉管炎(Thromboangitis Obliterans,TAO)的诊断价值。方法 31例临床确诊为血管闭塞性脉管炎患者,超声探测出42条病变血管,比较50条正常组相应动脉血管,对其二维声像图及血流动力学指标进行对比。结果 TAO的血管壁增厚,〉1.1mm,与正常组比较,差异有统计学意义(P〈0.05)。病变血管内膜面不光滑,管壁呈节段性不均匀性增厚,管腔内可探及斑片样稍强回声。频谱峰值血流速度和舒张末期血流速度均有不同程度的增加,但以舒张末期血流速度增加尤为显著,因此阻力指数减低。结论彩色多普勒血流显像(Color Doppler Flow Imaging,CDFI)可以对TAO进行定性、定量和血流动力学分析,且简便、经济、可靠,重复性强,可作为诊断TAO的首选方法。

Abstract:
Objective To probe the diagnosis value of color doppler ultrasonic image in thromboangitis obliterans(TAO).Methods 31 patients of TAO were clinic definited,42 vessels checked pathological changes by color Doppler flow imaging(CDFI),50 healthy people were recruited as the contrasted.Compared their 2-D ultrasonic imaging features and hemodynamics index.Results The vessle wall of TAO were thickening than nomal group,there were statistical significance(P〈0.05).In TAO group the endangium were not smooth,vessle wall were unhomogeneity,and found patching slighty dense echo in Lumen of vessel.The value of peak flow velocities in systolic and diastolic final phases increased,but the value of peak flow velocities in diastolic increased significant,So the resistance indexes(RI) decreased.Conclousion With color doppler flow imaging(CDFI),we can analysis haemodynamics changes of TAO,and convient,economy,reliability,better repetitiveness,it can be a first device in TAO diagnosis.
