软组织恶性纤维组织细胞瘤的MRI表现

目的:探讨软组织恶性纤维组织细胞瘤的MRI表现。方法:回顾性分析12例经手术病理及免疫组化证实的恶性纤维组织细胞瘤患者的临床、MRI及病理资料,12例均行MRI平扫加增强检查。结果:12例肿瘤的MRI表现:T1加权像上9例呈稍低信号或等信号,呈混杂信号3例;T2加权像均呈混杂高信号,7例肿瘤内可见低信号分隔征,5例瘤周可见假包膜,3例瘤周可见水肿;增强扫描肿瘤实性部分多明显强化,囊变、坏死区无强化。肿瘤最大径约5.3～16.9 cm,边界清楚10例。结论:MRI对软组织恶性纤维组织细胞瘤的诊断具有重要价值,确诊需依靠病理检查。

The MRI findings of malignant fibrous histiocytoma

Objective:To investigate the Magnetic Resonance Imaging(MRI) findings of malignant fibrous histiotoma.Methods: Twelve cases of malignant,fibrous histiotoma proved by pathology or Immunohistochemistry were analysed retrospectively.All of the cases underwent MRI plain scan and enhanced scan.Results: MRI showed slightly lower intensities or isointensities in nine cases and inhomogeneous intensities in three cases on T1 weighted images,and showed inhomogeneous high intensities on T2 weighted images.There were lower intensities compartments in seven cases,pseudocapsules in five cases and peritumoral edema in three cases.In the enhanced scans,there were apparent enhancement of the solid parts of tumors,and no enhancement in cystic and necrotic parts of the tumors.The Maximum diameter of tumors ranged from 5.3～16.9 cm.The borders were clear in 10 cases.Conclusion: MRI can be very helpful for the diagnosis of malignant fibrous histiotomas,but it still relies on pathology to make a definite diagonosis.
