人工晶状体植入术后迟发性葡萄膜炎发病机制探讨

摘    要:目的 观察后房人工晶状体植入术后循环免疫复合物的变化过程,探讨迟发性急性葡萄膜炎的发病机制。方法 成年大耳白兔20只,完成右眼透明晶状体乳化吸出囊袋内人工晶状体植入术,分别在术前及术后不同时间采耳静脉血,用聚乙二醇比浊法测定循环免疫复合物;并在裂隙灯下观察眼内炎症反应情况。结果 术后循环免疫复合物显著升高(t′=15.07,P<0.01),并在术后2~4周内维持较高水平。早期服内炎症反应在术后2周内基本消失。结论 后房人工晶状体植入术后迟发性急性葡萄膜炎是由免疫复合物介导的急性葡萄膜炎症反应。

Abstract:Objective To study the changes of circulating immune complex (CIC) and investigate the mechanism of late acute uveitis after cataract extraction with the implantation of posterior chamber intraocular lens (PC - IOL). Methods 20 experimental rabbits were treated with pha-coemulsification and implanted PC - IOL. Before operation and the seventh , fourteenth, twenty - first, twenty - eight and thirty - fifth days after the operation, CIC was determined with polgethylene glycol comparison methods,and the inflammatory reaction of the eyes was observed under slitlamp.Results After the operation in rabbit eyes,CIC rose markedly (t' = 15.07 P<0.001),and the level of CIC kept at high level in 2 - 4 weeks. Early inflammatory reaction disappeared in 2 weeks. Conclusion Late acute uveitis after the surgery is caused by immune complex (IC).
