门静脉血K-ras和p53基因突变与胃癌肝转移的关系

目的探讨门静脉血K-ras、p53基因突变与胃癌肝转移的关系。方法应用PCR单链构象多态性分析技术,对胃癌患者的门静脉血、外周静脉血分别进行K-ras、p53基因检测,术后随访半年,了解肝转移情况。结果62例胃癌患者外周血和门静脉血中的p53和K-ras基因突变阳性率分别为8.06%、4.83%和38.71%、33.87%;有无肝转移者p53和K-ras基因突变阳性率分别为92.30%、76.92%和24.49%、22.45%;剖腹后探查前或胃癌探查后门静脉血p53和K-ras基因突变阳性率分别为38.71%、33.87%和56.45%、62.90%,上述两组指标之间相比差异均有统计学意义(P<0.05)。结论检测门静脉血K-ras、p53基因突变对预测胃癌肝转移,判断预后有重要意义。

Detection of K-ras and p53 gene mutation in portal vein blood and its relationship with hepatic metastasis from gastric carcinoma

Objective To explore the relationship between K-ras and p53 gene mutation in portal vein blood and hepatic metastasis from gastric carcinoma.Methods By PCR-SSCP K-ras and p53 gene mutation in portal vein blood and peripheral blood were detected in 62 patients with gastric carcinoma.Results In 62 patients,p53 and K-ras mutation rates were 38.7% and 33.9% in samples from portal venous blood,but only 8.1% and 4.8% in peripheral blood.The rates of p53 and K-ras mutation were 24.5% and 22.5% before hepatic metastasis,92.3% and 76.9% after hepatic metastasis.The rates of p53 and K-ras mutation in portal vein were 38.7% and 33.9% before surgery,but 56.5% and 62.9% after surgery(P<0.05).Conclusion Detection of K-ras and p53 gene mutation in portal vein blood can help to predict hepatic metastasis in patients with gastric carcinoma.
