原发性高血压并发缺血性脑卒中与颈动脉粥样硬化的关系分析

目的:探讨颈动脉粥样硬化与原发性高血压(essential hypertension,EH)并发缺血性脑卒中的相关性。方法:选取82例EH患者为EH组,根据是否合并缺血性脑卒中分为单纯EH组(n=40)与EH并发缺血性脑卒中组(n=42),选取血压正常者80例为对照组。超声检测EH组与正常组的颈动脉内-中膜厚度、粥样硬化斑块面积及斑块回声强度。结果:EH组颈动脉内-中膜厚度、粥样斑块面积、斑块回声强度均明显大于对照组(P<0.05);高血压越严重,斑块面积越大,颈动脉的狭窄程度越高(P<0.05);单纯EH组的颈动脉斑块大于EH合并缺血性脑卒中组,内-中膜厚度小于EH合并缺血性脑卒中组(P<0.05)。结论:EH患者的颈动脉粥样硬化程度高于血压正常者,且EH合并缺血性脑卒中患者的颈动脉硬化程度更高。

Analysis of relationship between essential hypertension complicated with ischemic cerebral stroke and carotid atherosclerosis

Objective:To explore the relationship between carotid atherosclerosis and essential hypertension (EH) complicated with ischemic cerebral stroke.Methods:A total of 82 EH patients were selected as EH group and divided into single EH group (n =40) and EH complicated with ischemic cerebral stroke group (n =42) based on whether they were complicated with ischemic cerebral stroke.Another 80 people with normal blood pressure served as control group.Ultrasound was used to detect the carotid intima-media thickness,atherosclerotic plaque size and plaque echo intensity in EH group and normal group.Results:EH group was evidently higher than control group in carotid intima-media thickness,atherosclerotic plaque size and plaque echo intensity (P ＜ 0.05).The severer the hypertension was,the larger the plaque size and the higher then stenosis of carotid artery became (P ＜ 0.05).Single EH group was markedly higher in carotid atherosclerotic plaque and notably smaller in carotid intima-media thickness than EH complicated with ischemic cerebral stroke group (P ＜ 0.05).Conclusion:Patients with EH are higher than those with normal blood pressure,but lower than those with EH complicated with ischemic cerebral stroke in carotid atherosclerotic severity.
