老年男性2型糖尿病患者的静息能量消耗测定

摘    要:目的了解老年男性2型糖尿病患者静息能量消耗(RestingEnergyExpenditure,REE),为个性化饮食和控制体重管理提供依据。方法老年男性患者160例,年龄60—93(77.44±9.0)岁,身高157—180(169.8±4.73)cm,体质量50—102(70.78±9.39)kg,体质量指数(BMI)为18.6—36.1(24.5±2.91)kg/m2,其中2型糖尿病患者110例,同期非糖尿病患者50例为对照组。采用可携带式MedGem间接能量测量仪,按BMI〈24kg/m2,BMI24—27.9kg/m2和BMI≥28kg/m2分为3组,按Harris—Benedict公式计算REE。结果老年男性糖尿病患者实际REE测量值与非糖尿病比较增加4.2%。糖尿病患者按BMI分组后,REE实测值和预测值均随BMI增加而逐渐增高(P〈0.05),REE实测值和H—B公式预测值与体质量的相关系数分别为0.294和0.908(P〈0.05)。结论血糖控制差者其能量消耗增加。可携带式MedGem间接能量测量仪方法简便,对指导个体化饮食和血糖控制有临床实用意义。

Resting energy expenditure in aged male patients with type 2 diabetes mellitus

Abstract:Objective To investigate the resting energy expenditure(REE) in aged patients with type 2 diabetes mellitus in order to provide the theoretical basis for individualized diet and body weight control. Methods One hundred and sixty male patients were enrolled. Their age, body height, body weight, and BMI were 60-93 years(range, 77.44 ± 9.0 years), 157-180cm(range, 169.8 ± 4.73cm), 50-102kg(range, 70.78 ± 9.39kg), and 18.6-36.1kg/m2(24.5 ± 2.91kg/m2), respectively. The patients were divided into type 2 diabetes mellitus group(n=110) and non-diabetes group serving as a control group(n=50). REE was assayed with a portable indirect calorimeter, MedGem, by dividing the BMI of diabetes patients into〈24kg/m2, 24-27.9kg/m2 and /〉 28kg/m2, and calculated following the prediction equation of Harris-Benedict. Results The actual measured REE value in aged male patients with diabetes was 4.2%, which was higher than that in non-diabetes group. Both the actual measured REE value and the predictive value increased with BMI in diabetes patients(P〈0.05). The correlation coefficient of the actual measured REE value and the predictive value to the body weight was 0.294 and 0.908, respectively(P〈0.05). Condusion Energy consumption increases in those who cannot effectively control their blood sugar level. Portable MedGem indirect calorimeter is easy to apply and plays an important role in direction of individualized diet and in control of blood sugar level.
