血清肿瘤标记物联合检测辅助鉴别良恶性结肠疾病

目的血清癌胚抗原（CEA）、碱性磷酸酶（ALP）、CA125、CA19-9、CA72-4在结肠疾病鉴别诊断中的价值。方法应用Roche电化学发光免疫分析技术检测肿瘤标志物,全自动生化分析仪检测ALP,经病理学和影像学确诊的43例结肠息肉、88例结肠癌以及66例健康体检者入选本研究。结果在结肠癌组中,CEA、CA125、CA19-9、ALP水平均明显高于结肠息肉组和健康对照组（P〈0.05）,结肠癌组CA72-4与健康对照组比较有统计学差异（P〈0.01）。单一CEA、CA125、CA19-9、CA72-4检测鉴别诊断结肠癌特异性均较好,但敏感性均较低分别为35%、7%、11%、14%;CEA、CA19-9、CA72-4三者联合检测具有较好的敏感性和特异性。结论 CEA、CA19-9、CA72-4的联合检测对结肠癌的诊断及疗效观察具有较大价值,可提高结肠癌诊断的敏感性和准确性。

Differential diagnosis of benign and malignant colon diseases with serum tumor markers

Objective To study the value of tumor markers including CEA,CA125,CA19-9,CA72-4 and ALP in differential diagnosis of colon diseases.Methods Tumor markers and ALP were detected in 43 patients with colon polyps and 88 patients with colon cancer confirmed by pathology and imaging as well as in 66 healthy controls using ROCHE electrochemical luminescence immunoassay techniques and automatic biochemical analyzer,respectively.Results The CEA,CA125,CA19-9 and ALP levels were significantly higher in patients with colon cancer than in those with colon polyps and in healthy controls（P0.01）.The CA72-4 level in patients with colon cancer and in healthy controls was significantly different（P0.01）.The specificity of single CEA,CA125,CA19-9 and CA72-4 was higher in diagnosis of colon cancer with a lower sensitivity of 35%,7%,11% and 14%,respectively.The sensitivity and specificity of combined CEA,CA19-9 and CA72-4 test were higher.Conclusion Combined CEA,CA19-9 and CA724 test can increase its sensitivity and accuracy in diagnosis of colon cancer,thus having a greater value in its diagnosis.
