荧光原位杂交hTERC基因扩增检测在宫颈癌早期诊断中的应用

摘    要:目的探讨荧光原位杂交技术检测hTERC基因在子宫颈上皮内瘤变(CIN)和宫颈癌中的烤贝数扩增情况,了解其在宫颈癌早期诊断应用。方法收集2008年6月至2010年6月在本院就诊的396例宫颈脱落细胞,以生理盐水制片和液基细胞学(TCT)低渗法制片,应用荧光标记探针GLP TERC/CSP 300 m,用荧光原位杂交(FISH)技术进行hTERC基因烤贝数的检测,并分析检测结果的临床效应性。结果在396例临床标本中,正常宫颈细胞中表达率为1.012%(阈值);宫颈癌组的阳性率为93.65%(59/63)、CIN总阳性率为65.82%(104/158),宫颈癌组分别与正常组和炎性反应/CINⅠ组3.01%(4/133)相比较,差异均有统计学意义(P0.01);CINⅡ组的阳性表达率为60.24%(50/83),CINⅢ组的阳性率为72.0%(54/75),CINⅡ组、CINⅢ组分别与正常和炎性反应/CINⅠ组比较,差异均有统计学意义(P0.01);CINⅢ组、CINⅢ组与宫颈癌组比较,差异也均有统计学意义(P0.01);hTERC基因在宫颈癌细胞株hela和正常骨髓淋巴细胞中基因扩增呈阳性。结论 hTERC基因参与宫颈上皮内瘤变和宫颈癌的发生发展,作为宫颈上皮内瘤变及宫颈癌的标志物,hTERC的阳性表达率与宫颈细胞癌前病变具有严重程度相关。hTERC基因的检测可作为提高宫颈癌早期诊断率的重要手段和癌前病变治疗的重要依据。

Abstract:Objective To investigate the amplification of hTERC gene detected by fluorescence in situ hybridization technique in cervical intraepithelial neoplasia(CIN)and cervical cancer and to understand its application to early diagnosis of cervical cancer.Methods Cervical exfoliated cells in 396 cases in our hospital from June 2008 to June 2010 were collected,treated with normal saline and TCT.To use fluorogenic mark probe 300 of GLP TERC/CSP and fluorescence in situ hybridization(FISH)for detecting hTERC gene copies.The clinical effect of detection results was analyzed.Results In 396 clinical samples,the expressing rate was 1.012% in the normal cervical cells(threshold value).The positive rate of the cervical cancer group was 93.65%(59/63),the CIN total positive rate was 65.82%(104/158),the difference had statistical significance compared with normal and inflammation / CINⅠ group(3.01%(4/133)respectively(P〈0.01).The positive expression rate of group CINⅡ was 60.24%(50/83),the positive rate of CIN Ⅲ was 72.0%(54/75);comparing CINⅡand CINⅢ groups with cervical cancer group,the difference also had statistics significance(P〈0.01).The amplification of hTERC gene showed positive in cervial cancer cell lines hela and normal bone marrow lymphocytes.Conclusion HTERC gene participates in the occurrence and development of CIN and cervical cancer,as their marker,the positive expression rate of hTERC is related with the degree of precancerosis of cervical cells.The detection of hTERC gene may be as the important measure to increase the early diagnostic rate of cervical cancer and provide the important basis for treating precancerosis.
