曲美他嗪联合卡维地洛治疗老年充血性心力衰竭临床观察

目的观察曲美他嗪(TMZ)联合卡维地洛治疗老年充血性心力衰竭(CHF)患者的临床疗效。方法选择老年慢性CHF患者66例,随机分为两组:对照组(33例)给予洋地黄类、利尿剂、血管扩张剂、血管紧张素转换酶抑制剂(ACEI)、β受体阻滞剂(美托洛尔)常规治疗;研究组(33例)在常规治疗基础上用卡维地洛(10mg qd×60d)替换美托洛尔并联用TMZ(20mg tid×60d)治疗。结果研究组心功能改善的临床显效率(45.5%)和总有效率(93.9%)均较对照组(24.2%和66.7%)显著提高(P<0.01),且无不良反应出现。治疗后与治疗前相比,研究组心率、血压、左心室射血分数、左心室舒张末期容积、左心室收缩末期容积、每搏量、心排血量均有显著改善(P<0.01或P<0.05),研究组与对照组比较差异有显著性意义(P<0.01或P<0.05)。结论TMZ联合卡维地洛治疗老年CHF是一种安全有效的方法。

Effect of trimetazidine combined with carvedilol in the treatment of senile congestive heart failure

Objective To observe the effect of trimetazidine(TMZ) combined with carvedilol on the treatment of senile congestive heart failure(CHF).Methods 66 patients with CHF randomly divided into the treatment group(TMZ and carvedilol plus routine therapy n=33) and the control group(routine therapy n=33).Results The treatment efficiency of the treatment group and the control group were 93.9% and 66.7%(P<0.01).No adverse reaction related to the drug were observed.The improvements of heart rate,blood pressure,left ventricular ejection fraction,left ventricular end of diastolic volume,left ventricular end of systolic volume,cardiac output,stroke volume,after treatment in the treatment group were significant(P<0.01 or P<0.05).The difference between the treatment group and the control group was also significant(P<0.01 or P<0.05).Conclusion TMZ plus carvedilol with routine therapy may be a safe way to treat senile CHF.
