颈动脉体瘤的DSA诊断和术前栓塞

摘    要:目的探讨颈动脉体瘤的数字减影血管造影的影像特点和手术前栓塞的意义。方法回顾性分析1995年至2006年我院收治的23例手术前经导管栓塞的颈动脉体瘤患者的相关临床和影像资料,并与1954年至1995年收治的26例单纯手术切除的颈动脉体瘤患者作对比研究。结果颈动脉体瘤的特征性DSA表现:瘤体位于颈总动脉分叉处,颈内外动脉受推压移位夹角增大,瘤体血运丰富。术前栓塞安全、可行,与单纯手术组相比术中出血减少(P〈0.01),术程有所缩短,并发症减少,肿瘤一期切除。结论颈动脉体瘤的DSA表现特征明显,对诊断、鉴别诊断和手术治疗方案的选择至关重要,术前栓塞具有极大的临床价值。

Abstract:Objective To evaluate the DSA characters of carotid body tumors and the importance of preoperative transcatheter embolism. Methods Twenty - tyree cases of carotid body tumors with preoperative embolism were retrospectively studied and analyzed. Additionally, the intraoperative blood loss, operative time and postoperative complications were compared with 26 cases without preoperative embolization. Results The localization of the tumor within the carotid bifuracation, splaying of the bifuracation and profuse vascularity were the points in the diagnosis of carotid body tumor by DSA. Transcatheter superselective arterial embolizations before surgery could reduce the chance of hemorrhage in the operation, shorten operative time, simplify the operative procedure and avoid the serious postoperative complications. Conclusion The DSA appearance of carotid body tumor is the most important modality of diagnosis, differential diagnosis and choice for treatment plan. Embolization before resection has great clinical values.
