临床路径在我院结肠癌术后辅助化疗中的应用分析

目的 分析临床路径在医院结肠癌术后辅助化疗患者中的应用情况.方法 将肿瘤科80例结肠癌术后(高危Ⅱ期/Ⅲ期)辅助化疗患者纳入研究.随机分为临床路径组(研究组)与对照组,对临床路径在结肠癌术后辅助化疗患者中的应用进行比较研究.结果 与对照组相比.临床路径组惠者的住院天数减少了1.1 d(P<0.05),费用降低了3 017.60元(P<0.05).在药物应用方面,指南推荐的主要治疗药物两组无显著差异.但相同方案临床路径组中应用进口奥沙利铂的患者药品费用明显高于应用国产奥沙利铂患者(P<0.01).结论 应用临床路径可有效缩短结肠癌术后辅助治疗患者的住院时间,降低总住院费用及药物费用.为进一步降低患者的治疗费用,今后还应对进口和国产奥沙利铂进行成本一效果分析和不良反应评价,以期为优选治疗药物、促进该病种临床路径的进一步改进提供依据.

Application Analysis of Clinical Pathway in Patients with Adjuvant Chemotherapy after Resection of Colon Cancer

Objective To investigate the application status of clinical pathway in the patients with adjuvant chemotherapy after resection of colon cancer.Methods Eighty inpatients with adjuvant chemotherapy after resection of colon cancer（high-risk stage Ⅱ/Ⅲ） were included in this study and randomly divided into the clinical pathway group and the control group.And the comparative study was performed on the data in the clinical pathway application.Results Compared with the control group,the hospitalization days after conducting the clinical pathway in the clinical pathway group were decreased by 1.1 d（P〈 0.05） and the costs were decreased by 3 017.60 Yuan（P 〈0.05）.There was no significant difference in the treatment programs recommended by guideline between the two groups.But the costs for using imported oxaliplatin were much higher than those for domestic oxaliplatin in the clinical pathway group（P 〈0.01）.Conclusion The clinical pathway can effectively shorten the patients′hospitalization time and reduce the total hospitalization costs and drug costs,which is worthy of being popularized.To further reduce patients′costs,the cost-effectiveness analysis and the evaluation of adverse reactions should be performed to provide the basis for optimizing the treatment drugs and promoting further improvement of clinical pathway for this disease.
