换血治疗早产儿重度高胆红素血症94例临床分析

[目的] 探讨换血术治疗早产儿重度高胆红素血症（以下简称重度高胆）的效果.[方法] 采用经外周动静脉同步换血疗法对94例重度高胆早产儿进行治疗,比较换血前后血常规、血电解质和胆红素的差别,跟踪随访脑干听觉诱发电位（BAEP）的变化情况.[结果] 94例重度高胆早产儿总胆红素换出率为66.92%,血清总胆红素换血前的均值（495.45±154.43）μmol/L与换血后的均值（163.88±66.27）μmol/L相比较,差别有统计学意义（ P 〈0.05）.换血后血白细胞、红细胞、血红蛋白、血小板、血钙、血钾及血氯与换血前相比较,均存在显著性差别（ P 〈0.05）.换血并发症较少,临床症状明显改善.[结论] 换血治疗早产儿重度高胆红素血症安全有效.

Clinical Analysis of Blood Exchange Transfusion on 94 Premature Infants with Severe Hyperbilirubinemia

[Objective] To investigate the clinical effect of blood exchange transfusion on premature infant with severe hyperbilirubinemia. [Methods]Blood exchange transfusion was performed on 94 premature infants. The difference of blood routine, blood electrolyte and bilirubin before and after transfusion was compared. The changes of brain-stem auditory evoked potential（BAEP） were followed up. [Results]The mean drop rate of serum total bilirubin was 66.92%. The serum total bilirubin concentrations before and after trans- fusion were （495.45±154.43）μmol/L and （163.88± 66.27）μmol/L, respectively, and there was statistical difference（ P 〈0. 05）. There were significant differences in blood leucocyte, red blood cell, hemoglobin, platelet, blood calcium, kalium and chlorine between before and after transfusion（ P d0.05）. After transfusion, the complications decreased and the clinical symptoms improved obviously. [Conclusion]Blood exchange transfusion for the treatment of premature infants with severe hyperbilirubinemia is safe and effective.
