| 摘 要： | 目的探讨老年患者十二指肠乳头切开取石术（endoscopic sphincterotomy,EST）术后发生急性坏疽性胆囊炎（acut egangrenous cholecystitis．AGC）的围术期护理。方珐回顾性分析2003年6月至2007年1月我院收治的1066例行ERCP＋EST患者的临床病理资料,对其中的16例老年AGC患者的围术期护理措施进行回顾性分析。结果16例老年患者ERCP＋EST术后并发AGC,发生率为1．5％。16例老年患者经过手术治疗和围术期精心护理均成功治愈,无死亡病例。结论AGC患者术前可发生胆囊穿孔、严重全身感染等情况;外科手术操作困难,手术中容易误伤胆道系统和肠管;术后发生胆瘘和出血的风险相对较高,早期外科手术和围术期并发症的密切观察和护理是保证AGC成功治愈的关键。
