肺炎克雷伯菌对季铵盐消毒剂及复方新诺明耐药相关基因检测

摘    要:
目的了解临床分离的肺炎克雷伯菌(KPN)季铵盐消毒剂及复方新诺明耐药相关基因存在状况。方法在2005年9月~2006年4月从住院患者中分离出25株肺炎克雷伯菌,采用PCR及序列分析的方法分析5种基因qacE△1-sul1、dfrA1、dfrA5、dfrA12和dfrA17。结果25株肺炎克雷伯菌中,qacE△1-sul1、dfrA1、dfrA5、dfrA12和dfrA17基因阳性株数分别为21(84.0%)、0、6(24.0%)、6(24.0%)和0;qacE△1-sul1阳性基因测序比对与已在美国核苷酸序列数据库(GenBank)中登录的qacE△1-sul1基因序列完全同源。结论临床分离的肺炎克雷伯菌中qacE△1-sul1基因阳性率很高,对复方新诺明耐药与菌株携带dfrA5、dfrA12和qacE△1-sul1基因有关。

Abstract:
OBJECTIVE To investigate the presence of the quaternary ammonium compounds-resistant gene and sulfamethoxazole/trimethoprim-resistant genes in Klebsiella pneumoniae isolated from the PLA 98th Hospital, Huzhou,Zhejiang province.METHODS Twenty-five strains of K.pneumoniae were isolated from the inpatients from Sep 2005 to Apr 2006.Five kinds of genes of qacE△1-sull,dfrAl,dfrA5,dfrA12 and dfrA17 were analyzed by PCR and verified by DNA sequencing.RESULTS In 25 strains of K.pneumoniae, the positive rate of genes of qacE△1-sull, dfrA5 and dfrA12 were 84.0%,24.0%,and 24.0%,respectively.Both genes of dfrAl and dfrA17 were tested negative.The total positive rate of 5 kinds of genes was 92.0% (23/25).qacE△1-sull Positive gene sequencing showed highly homology with the sequence of GenBank (GenBank Accession Number; U12338).CONCLUSIONS There is very high positive percentage of qacE△1-sull gene in K.pneumoniae isolated from the inpatients.There is a close relation of the sulfamethoxazole/trimethoprim resistance in K.pneumoniae to qacEAl-sull,dfrA5 and dfrA12 existing.
