彩色多普勒对下肢动脉硬化闭塞症的超声诊断价值

目的探讨彩色多普勒对下肢动脉硬化闭塞症的诊断及价值。方法用PHILIPS5500及IE33超声诊断仪,探头频率L11～3MHz,对33例下肢动脉硬化闭塞症患者进行检查,观察其二维图像、患者下肢动脉的内径、是否有斑块形成、彩色多普勒血流特点及频谱特征。结果下肢动脉硬化闭塞症患者中,二维显示管腔内-中膜增厚,斑块形态各异、大小不一;彩色多普勒血流信号充盈缺损,管腔狭窄处彩色血流变细、血流加快,可见多彩镶嵌血流,完全阻塞时血流中断,信号消失不能显示。结论彩色多普勒超声检查对下肢动脉硬化闭塞的超声诊断的敏感性、准确性较高、无创伤,可以作为诊断动脉硬化闭塞症、为临床提供可靠依据的首选检查方法。

Diagnostic Value of Arteriosclerosis Obliterans of the Lower Limbs Using Color Doppler Ultrasound

Objective To explore the diagnosis and value of the arteriosclerosis obliterans of the lower limbs using color Doppler ultrasonography.Method 33 patients suffered from arteriosclerosis obliterans of the lower limbs were examined by PHILIPS5500 and IE33 color Doppler instrument,which probe＇s frequency was L11 ～ 3 MHz.The two-dimensional graphs,the inner diameter of the vessels,the formation of the plaques,the characteristics of the blood flow and the frequency spectrum were observed.Result Two-dimensional ultrasonography indicated the intima and tunica media became thicker and there were different shapes and sizes of the plaques,color Doppler flow imaging（CDFI） indicated that there was the filling defect of the blood flow signal,color blood flow became thinner and faster,colorful mosaic blood flow could be seen at the place of stenosis.And there was interruption and no signal of blood flow when the lumen was completely obstructed.Conclusion Color Doppler ultrasonography is a sensitive,accurate and non-invasive way to diagnose arteriosclerosis obliterans of the lower limbs.And it can be the first choice to provide reliable evidence for the clinic in the diagnosis of arteriosclerosis obliterans.
