审慎诊治脉络膜恶性黑色素瘤

目的探讨脉络膜恶性黑色素瘤的传统治疗、影像学临床特点及确诊符合率,以指导临床。方法经临床确诊为脉络膜恶性黑色素瘤患者26例26眼,年龄41～73岁。术前视力0.2至光感。局部及全身检查除外肿瘤原发病灶及转移病灶。结果26例行眼球摘除术,病理检查结果符合率为88.46%(23/26)。3例3眼病理检查结果为脉络膜出血。结论审慎诊治脉络膜恶性黑色素瘤,提高确诊率,现代的包括手术在内的治疗方法应加以考虑。

Diagnosing and treating the choroidal melanoma prudently

Objective To investigate retrospectively the traditional therapy for the choroidal melanoma and its imaging characteristic and the coincidence rate with pathology.Methods Twenty-six cases(26 eyes)were diagnosed as choroidal melanoma in clinic.Age was from 41 to 73 years old.Before operation,the vision was 0.2 to light reception.Original tumor lesion and metastasis tumor lesion in system were excluded by local and systemic examination.Results Twenty-six eyes were enucleated and diagnosed as choroidal melanoma pathologically 88.46%(23/26),3 eyes were diagnosed as choroidal hemorrhage pathologically.Local tumors looked like semi-sphere or mushroom toward vitreous were detected in 19 eyes;Local tumors grown irregularly were detected in 7 eyes.There were "dig phenomenon" companied with ultrasonic shadow in 14 eyes;choroidal concave in 18 eyes;complicated by retinal detachment in 10 eyes.Conclusions Choroidal melanoma should be diagnosed and treated carefully in order to improve the exact diagnosing rate,and save the eyeball and vision by the modern therapy if it is possible.
