气压弹道和输尿管软镜钬激光治疗上尿路结石的临床疗效对比研究

目的 对比分析气压弹道和输尿管软镜钬激光治疗上尿路结石的临床疗效.方法 选择适合应用输尿管软镜的上尿路结石患者80例,根据治疗方法的不同分为观察组与对照组各40例,观察组采用输尿管软镜钬激光碎石术,对照组采用气压弹道式碎石术.结果 观察组的手术时间和术后住院时间明显少于对照组(P＜0.05),但观察组的总医疗费用高于对照组(P＜0.05).观察组的结石清除率明显高于对照组.随访1个月,观察组的结石排净率也明显高于对照组,差异都有统计学意义(P＜0.05).随访3个月,两组严重出血、输尿管穿孔、感染性休克等总体并发症发生率对比无明显差异(P＞0.05),观察组的满意度为95.0％,对照组满意度为75.0％,对比差异有统计学意义(P＜0.05).结论 输尿管软镜联合钬激光碎石治疗上尿路结石能提高碎石成功率,对于患者的创伤小,患者满意度高,值得推广应用.

The comparative study of the traditional pneumatic ballistic lithotripsy and flexible ureteroscope holmium laser lithotripsy in the treatment of upper urinary tract calculi

Objectives To comparative analysis the clinical effects of the traditional pneumatic ballistic lithotripsy and flexible ureteroscope holmium laser lithotripsy in the treatment of upper urinary tract calculi.Methods Eighty patients who underwent flexible ureteroscope lithotripsy were randomly divided into observation group and control group,the patients in the observation group underwent flexible ureteroscope holmium laser lithotripsy,and the patients in the control group were given traditional pneumatic ballistic lithotripsy.Results The operative time and postoperative hospital stay were significantly less than the control group (P ＜ 0.05),but the total medical cost of the observation group was higher than that of the control group (P ＜ 0.05).The stone clearance rate of the observation group was significantly higher than the control group Followed-up 1 month,the stone-free rate of the observation group was significantly higher than the control group (P ＜ 0.05).Followed-up 3 month,the severe bleeding,ureteral perforation and septic shock overall complication rate between the two groups had no significant difference (P ＞ 0.05).There was statistically significant difference in the satisfaction between the observation group(95.0％) and control group(75.0％,P ＜ 0.05).Conclusions Flexible ureteroscope holmium laser lithotripsy in the treatment of upper urinary tract calculi can improve the success rate of lithotripsy with less trauma and high satisfaction,it is worth clinical promotion.
