青年人胃癌40例的临床,内镜及病理分析

４０例青年人胃癌，占同期我院胃镜检出胃癌６９９例的５．７％。男性２６例，女性１４例。３１～３５岁为１５例（３７．５％），最小年龄为１５岁。早期胃癌３例（７．５％），进展期癌３７例。手术切除率为４２．５％。５年生存率为１５％（６／４０）。本组发现５例胃癌与十二指肠溃疡共存。本文分析了青年人胃癌的特点与预后差的主要原因，并讨论了青年人胃癌与胃溃疡关系以及与十二指肠溃疡共存问题。

CLINICAL,ENDOSCOPIC AND PATHOLOGICAL ANALYSIS OF FORTY CASES OF GASTRIC CARCINOMA IN YOUNG ADULTS

Forty cases of gastric carcinoma in young adults(5.7%)were collected from a total of 699cases of gastric carcinoma detected by gastroscopy in a 20 year period at Beijing Hospital.Twenty-six of the patients were male and fourteen female.Fifteen cases(37.5%)were betweenthe ages of 31 to 35 years, and the youngest one was l5 years old,There were 3 cases(7.5%)of early gastric carcinoma and 37 cases of progressive carcinoma.The resectability rate was42.5%and 5 years survival rate l5%(6/40).In this series,five cases of gastric carcinomacoexisted with duodenal ulcer.The characteristics of gastric carcinoma in the young adultsand the main reasons for their poor prognosis were analysed.The relation of gastriccarcinoma to gastric ulcer in the young adults,as well as the coexistence of.gastriccarcinoma with duodenal ulcer,was discussed.It was emphasized that in order to improve thepoor prognosis,great attention should be paid to the early diagnosis of gastric carcinoma inyoung adult;gastric ulcers in young adults should be followed up closely by gastroscopy andpathology,and the clinicians must be alert to the coexistence of gastric carcinoma withduodenal ulcer.Gastric lesions should be evaluated carefully as if duodenal ulcer was not present.
