经食管三维超声心动图对主动脉瓣关闭不全合并升主动脉扩张患者主动脉根部结构的评价

摘    要:
目的利用经食管三维超声心动图评估中-重度主动脉瓣关闭不全合并升主动脉扩张患者的主动脉根部结构及其与左心室功能的关系。方法 15例存在中-重度主动脉瓣关闭不全合并升主动脉扩张患者为病例组,17例主动脉瓣结构正常的患者为对照组,行经食管三维超声心动图检查。eSie Valves分析软件获得主动脉根部相关参数,同时获取病例组左室功能参数。结果与对照组相比,病例组主动脉根部各径线均增大。病例组中,术前发现升主动脉内径与窦部、窦管交界内径具有相关性,术后升主动脉未见明显回缩。病例组二尖瓣环-主动脉瓣环夹角与术前左室射血分数呈明显负相关。结论能够利用经食管三维超声心动图评价主动脉瓣关闭不全合并升主动脉扩张患者的心内构型及解剖关系的变化,为术前评估以及术后再评价提供参考。

Abstract:
Objective To assess anatomic characteristic of the aortic root in patients with moderate-severe aortic regurgitation and dilated proximal ascending aorta using the 3 D-Transesophageal Echocardiography(3 D-TEE)and analyse the relationship between ventricular function and the aotic root.Methods 15 patients with moderate-severe aortic regurgitation and dilated proximal ascending aorta were the case group.17 patients with normal aortic valve were selected as the control group.All patients were examined by 3 D-TEE.The software eSie Valves automatically configured model of the aortic root and left ventricular function were obtained.Results Compared with the control group,the aortic root dimensions of case group were all increased.In the case group,there was a correlation between proximal ascending aorta dimension and sinotubular junction and sinus dimensions before AVR.There was no significant difference between the pre-and post-AVR proximal ascending aorta dimensions.Aortic-mitral angle(AMA)was negatively correlated with LVEF before AVR.Conclusions 3 D-TEE can ke wsed to evaluate the changes of intracardial structure and anatornical relationship in patients with aortic regurgitation and dilated proximal ascending aorta,prouiding a reference for preoperative waluation and postoperative reevaluatim.
