多层螺旋CT重建对静脉尿路造影上尿路显影 不佳疾病的诊断价值

摘    要: 【目的】 探讨多层螺旋CT(MSCT)重建技术对在IVU检查中上尿路显影不佳疾病的诊断价值?【方法】 分析46例IVU上尿路显影不佳病例的CT重建表现;46例IVU显影不佳病例,均行MSCT检查,在图像工作站对获得图像进行MIP?MPR?CPR?SSD及VR后处理,获得泌尿系三维图像?【结果】 尿路结石20例,先天性肾盂输尿管连接处狭窄7例,输尿管炎症狭窄3例,输尿管损伤合并尿漏1例,迷走血管压迫左侧输尿管2例,肾脏输尿管结核3例,输尿管下段移行细胞癌3例,输尿管下段憩室1例,小肾畸形4例(其中一例为马蹄肾合并右肾发育不良),2例双肾萎缩,以上病例均经手术?输尿管镜检查或临床证实?【结论】 MSCT重建对IVP上尿路显影不佳病变有着很高的临床应用价值,可使输尿管病变诊断准确率得到明显提高?

Abstract:Objective] To evaluate the role of multi-slice computed tomography (MSCT) in the diagnosis of the disease which had upper urinary tract seen unclearly in intravenous urography. Methods] To analyze CT reconstruction images of 46 patients with poor visualization in the intravenous urography. All those patients underwent MSCT plain scan. Original data were transmitted to the workstation,and 3D images of the urinary system with MIP, MPR, CPR, SSD, and VR were obtained. Results] Among the 46 cases,there were 20 cases of ureteral calculus,7 cases of ureteropelvis junction stenosis,3 cases of inflammatory stenosis,1 case of ureter injury with urine leakage,2 cases of ectopic vessel compression at left ureter,3 cases of ureter and kidney tuberculosis,3 cases of primary uretal cancer,1 case of lower ureter diverticulum,4 cases of small kidneys(1 case of horseshoe kidney with the right renal dysplasia),2 cases of both renals atrophy. All cases were confirmed by surgery,ureteroscopy and clinic. Conclusion] MSCT reconstruction after intravenous urography has high value in clinical application for the disease with poor visualization of the upper urinary tract in intravenous urography and improve accurate rate.
