鼻内镜手术治疗慢性鼻-鼻窦炎500例疗效分析

目的观察经鼻内镜手术治疗慢性鼻-鼻窦炎的临床疗效。方法对经鼻内镜手术治疗的500例慢性鼻-鼻窦炎患者均采用Messerklinger术式，术中根据病变程度决定钩突切除的多少和筛窦切除的范围，并同时纠正鼻腔结构异常；对临床疗效进行评价，并对并发症进行分析，术后随访1～3年。结果治愈267例（53．4％），好转198例（39．6％），无效35例（7％），总有效率为93％。并发症27例，发生率为5．4％。结论鼻内镜手术治疗慢性鼻-鼻窦炎效果良好。手术的关键是解除鼻腔、鼻窦的引流和通气障碍。术中应同时纠正鼻腔的各种解剖异常，重视钩突的生理功能，妥善处理围手术期的各种问题，有利于提高手术的疗效和减少并发症、降低复发率。

Ocurative effect of endoscopic surgery on chronic rhinosinusitis

Objective To observe the curative effect of endoscopic surgery on chronic rhinosinusitis. Methods 500 patients with chronic rhinosinusitis were treated by endoscopic surgery with Messerklinger method. The anatomical abnormality of nasal cavity was rectified. The patients were followed up for 1-3 years follow-up. The clinical effect and the complications were analyzed. Results there were 267 cases （53.4%） were cured and 198 cases （39.6%） improved and 35 cases （7%） ineffective. The total effective rate was 93%. The incidence of complications was 5.4%. Conclusion Is is a good effect to treat chronic rhinosinusitis with endoscopic surgery. The key point of this surgery is to release the dysfunction of draining and ventilation at nasal cavity and paranasal sinuses.
