Efficacy and safety of bilateral simultaneous total knee arthroplasty and unilateral staged total knee arthroplasty in the treatment of patients with knee osteoarthritis

Objective To investigate the efficacy and safety of bilateral simultaneous total knee arthroplasty and unilateral staged total knee arthroplasty in the treatment of patients with knee osteoarthritis.Methods The clinical data of 100 patients with knee osteoarthritis admitted in our hospital from January 2017 to January 2020 were retrospectively analyzed and divided into bilateral simultaneous group(bilateral simultaneous total knee arthroplasty,n=50)and unilateral staged group(unilateral staged total knee arthroplasty,n=50)by the operation method.The hemoglobin level,degree of pain,range of motion of knee joint,knee joint function,bone joint function,blood loss volume,blood transfusion volume,hospital stay,hospitalization expenses and perioperative complications were compared between the two groups.Results Compared with before operation,the decrease range of hemoglobin level in the unilateral staged group was significantly smaller than that in the bilateral simultaneous group,and the decrease range of VAS and WOMAC scores and the increase range of range of motion of knee joint and HSS score in the unilateral staged group were significantly larger than those in the bilateral simultaneous group,the differences were statistically significant(P<0.05).The postoperative blood loss volume and blood transfusion volume of the unilateral staged group were significantly less than those in the bilateral simultaneous group,but the hospital stay was significantly longer than that in the bilateral simultaneous group,the hospitalization expenses was significantly higher than that in the bilateral simultaneous group,the differences were statistically significant(P<0.05).The total incidence of perioperative complications in the unilateral staged group was 10.0%,which was significantly lower than 28.0%in the bilateral simultaneous group,the difference was statistically significant(P<0.05).Conclusion Compared with bilateral simultaneous total knee arthroplasty,unilateral staged total knee arthroplasty in the treatment of knee osteoarthritis has better effect and higher safety,which is worthy of promotion.
