硬核白内障手术方法比较

目的:评价超声乳化及小切口白内障手术对Ⅳ级硬核白内障的效果。方法:回顾性分析2008-01/07间81眼81例Ⅳ级硬核白内障手术的临床资料。Ⅳ级硬核白内障81例81眼白内障患者随机分为超声乳化组(n=41)和小切口组(n=40)。比较两组手术时间、手术费用、手术并发症及术后1d;1,3wk和3mo时的视力、角膜散光和中央角膜厚度等。结果:两组手术时间大致相同。超声乳化组手术费用明显高于小切口组。术后1d和1wk时小切口组视力好于超声乳化组,中央角膜厚度明显低于超声乳化组,差异有显著性(P<0.05)。两组术后3wk和3mo时视力和角膜散光无明显差别,中央角膜厚度正常。小切口组手术并发症更多。结论:小切口白内障手术经济实用,手术效果和超声乳化一致。

Comparison of surgeries for hard nuclear cataract

AIM:To study effect of phacoemulsification and small incision cataract extraction in treating Ⅳ grade hard nuclear cataract.·METHODS:Clinical data of cataract extraction for hard nuclear cataract in 81 eyes(81 patients) from January,2008 to July,2008 were studied retrospectively.A total of 81 cases(81 eyes) with Ⅳ grade hard nuclear cataract were randomly divided into phacoemulsification group(n=41)and small incision cataract extraction group(n=40).The operation time,operation complications,visual acuity,corneal astigmatism and central corneal thickness at 1 day,1 week,3 weeks and 3 months postoperatively were compared.·RESULTS:Significant difference existed between the two groups in corneal astigmatism and central corneal thickness 1 week after operation(P<0.05),but no significant difference at the third month postoperatively.No significant difference in post-operative visual acuity between the two groups 1 week and 3 months postoperatively.·CONCLUSION:The small incision cataract extraction is a safe and practical way in treating hard nuclear cataract and the effect is the same as phacoemulsification.
