糖化血红蛋白对急性冠脉综合征患者预后的影响

摘    要:
目的 研究糖化血红蛋白(HbA1c)与急性冠脉综合征(ACS)患者预后的关系.方法 根据冠脉造影结果,将68例ACS患者分为冠脉单支病变组(47例)和多支病变组(21例);按NYHA分级法Ⅱ级以上,将68例ACS患者分为心衰(HF)组(26例)和无HF组(42例).患者入院取肘静脉血测定血糖(AG),次日清晨空腹取静脉血测定HbA1c、空腹血糖(FPG).结果 多支病变组HbA1c和FPG水平均明显高于单支病变组(P<0.01),HF组HbA1c和FPG水平均明显高于无HF组(P<0.05).结论 HbA1c与ACS患者冠脉病变程度、心衰密切相关,并且是影响ACS患者预后的预测因子;将HbA1c控制于正常范围内,能有效降低ACS患者的冠脉病变程度及心衰的发生.

Abstract:
Objective To study the relationship between the blood contents of HbA_(1c) and the prognosis of the patients with acute coronary syndrome ( ACS). Methods According to the results of coronary arteriography, 68 cases were divided into multiple vessels groups (21 cases ) and single vessel group(47 cases) ;according to the levels of NYHA, 68 cases were divided into HF group (26 cases) and non - HF group(42 cases). HbA_(1c),FPG and AG were examined in 68 subjects. Results Among the ACS patients,the levels of HbA_(1c) ,FPG in the patients with multiple vessels were significantly higher than those in the patients with single vessels (P<0. 01) ,the contents of HbA_(1c) and FPG in the patients with heart failure ( HF) were significantly higher than those in the patients with non - HF( P <0. 05). Con-clusion Contents of HbA_(1c) are correlated with the severity of coronary artery and HF,and could predict the prognosis of patients with ACS; Severity of coronary artery and HF could be reduced if the levels of HbA_(1c) were controlled at the normal range.
