保乳手术治疗早期乳腺癌实效性研究

目的：探讨保乳手术治疗早期乳腺癌患者临床效果及美容价值，为临床选择术式提供参考。方法：选取2009年7月-2011年12月本院收治的42例早期乳腺癌患者，按手术方式不同将其分为观察组22例和对照组20例。对照组实施改良根治手术；观察组实施保乳手术。观察比较两组患者的临床指标、术后随访情况、美容满意度。结果：观察组的手术时间、住院时间均明显短于对照组，术中出血量明显少于对照组，差异均有统计学意义（P〈0．05），但两组并发症发生率、局部复发率和远处转移率比较差异均无统计学意义（P〉0．05）。对照组2例远处转移为脑部转移。观察组的美容满意度100％明显高于对照组的40．00％，差异有统计学意义（P〈0．05）。结论：保乳手术与改良根治手术均可以取得较好的临床效果，但保乳手术美容效果好，在严格掌握适应证的前提下，临床可以优先考虑。

The Research on the Effectiveness of Breast-conserving Surgery in Treatment of Early Breast Cancer

Objective： To investigate the clinical effects and cosmetic value of breast-conserving surgery for early breast cancer patients, to provide a reference for the clinical selection of surgery.Method： 42 patients with early breast cancer were selected from July 2009 to December 2011.According to surgical option, all patients were divided into the observation group for 22 cases and the control group for 20 cases.The control group was given modified radical surgery, the observation group was given breast-conserving surgery.The clinical indicators, postoperative follow-up situation, beauty satisfaction were observed and compared between the two groups.Result： The operative time, length of hospital stay in the observation group were significantly shorter than those in the control group, the blood loss was significantly less than the control group, the differences were statistically significant （ P〈0.05 ） .There were no significant differences in the complication rate, local recurrence rate and distant metastasis rate between the two groups （ P〉0.05 ） .The control group had 2 cases of distant metastasis for brain metastasis.The beauty satisfaction of the observation group was 100%, it was significantly higher than the 40.00% of the control group, the difference was statistically significant （ P〈0.05 ） . Conclusion： Both breast-conserving surgery and modified radical surgery can achieve good clinical results, but breast- conserving surgery is better in cosmetic, so breast-conserving surgery can be a clinically priority option.
