老年冠心病患者细胞免疫功能的研究

目的 探讨细胞免疫反应在老年冠心病(CHD)的发病机理及病程发展中的作用。方法 应用生物学活性检测方法定量测定了老年CHD患者外周血单核细胞产生IL-2的水平，同时以APAAP技术测定外周血T细胞及其亚群，把免疫细胞表型的变化与功能状态结合起来研究。结果与正常对照组比较，老年CHD患者外周血PBMC中CD3^ 、CD4^ 及CD8^ 细胞的百分率均较正常对照组减低，其中以CD8^ 细胞降低更为显著。IL-2产生水平在老年CHD组明显减低，其中以冠心病心力衰竭组降低更为显著。免疫调变剂消炎痛能显著增加老年CHD病人PBMC产生IL-2的能力。老年CHD病人CD4^ 细胞的百分率与PBMC产生IL-2的活性呈正相关。结论 免疫衰老及其它因素所致的免疫活性细胞功能和数量的变化可能与CHD的发病机制及病变的严重程度有关。

Studies on T cell,T cellsubsets and lymphokin activities of peripheral blood mononuclear cells in patients withcoronary heart disease

Objective To study the roles of cell immune reaction to pathogenesis of elderly coronary heart disease(CHD).Methods Quantitative measuring the concentration of interleukin-2 which produced by peripheral blood mononuclear cells(PBMC)of elderly CHD patients by bioactive measuring method,and determine peripheral blood T lymphocyte and it's subsets in PBMC by APAAP immunohistologic technique using monoclonal antibodies against human T lymphcyte differantiation antigens.Results In patients with CHD,the percentage of CD3 ,CD4 and CD8 cell and the concentration of IL-2 produced by PBMC significantly decreased compared with normal control.Among which the percentage of CD8 cells in PBMC decreased even more marked.Correlation analysis showed positive and significant parallelism between IL-2 activity and the percentage of CD4 cells in PBMC.Indomethacin can increase IL-2 production of PBMC in patients with CHD Conclusions The functions and numbers of T lymphocyte in PBMC decreased significantly in patients with CHD,in which the defection of CD8 cells is more prominent.These change could be related to senility and may deteriorate the disturbance of humoral immunity and promote the formation of atherosclerosis,at last related to the pathogenesis of CHD.
