280例阻塞性睡眠呼吸暂停综合征患儿睡眠呼吸监测结果及治疗探讨

目的分析、探讨儿童阻塞性睡眠呼吸暂停低通气综合征（OSAHS）的睡眠呼吸监测数据及诊断参考值。方法①回顾性分析280例OSAHS患儿及20名正常儿童的多导睡眠监测仪（PSG）检查结果，分别按呼吸暂停持续时间10s和6s进行数据分析，并对低通气指数（AHI）结果进行统计学处理；②根据患儿病史、查体、PSG检查结果及电话等方式随诊，了解AHI在儿童OSAHS的诊断、转归及预后的判断等方面的影响。结果①在轻、中度时，按10s的呼吸暂停标准与按6s的呼吸暂停标准分析，有显著差异（P〈0．05），重度时2者间无差异；②AHI≥5作为儿童OSAHS的诊断标准较为合适。结论①采用呼吸暂停持续时间10s与6s进行PSG结果分析，对于确立轻度OSAHS的诊断有较大差别，故对儿童应采用6s进行分析；②AHI≥5较为合适的儿童OSAHS的诊断标准，并且病史应超过半年以上，对AHI为2-4之间的儿童应进行随诊；③儿童OSAHS的疗效应根据治疗前后的AHI的变化来评判：

Sleep-breathing monitoring of 280 child cases with obstructive sleep apnea hypopnea syndrome

Objective Analysis of sleep-breathing monitoring data and its diagnostic significance for OSAHS. Methods （1）Retrospective analysis of the results of PSG in 280 childcases of OSAHS and 20 normal children, to do data analysis according to apnea duration of 10s and 6s respectively, and the AHI were statistically treated. （2）To assess the effects of AHI on diagnosis, outcome and judgment of prognosis. Results （1）In mild and moderate cases, there were significant differences （P〈0.05） in analysis with 10s and 6s apnea standard, but in severe cases there were no differences. （2）The suitable OSAHS diagnosis standard was AHI≥5. Conclusion （1）In mild cases of OS- AHS, we should analyse with 6s apnea parameter. （2）The suitable child OSAHS diagnosis standard is AHI ≥5.
