肥胖与非肥胖型多囊卵巢综合征临床对照性研究

目的探讨肥胖与肥胖型多囊卵巢综合征在生殖内分泌、糖脂代谢方面的差异。方法筛选PCOS患者60例,根据体重指数(BMI)分为肥胖组和非肥胖组,测定各项血清学指标。结果肥胖组血睾酮(T)水平高于非肥胖组,黄体生成素(LH)、LH/卵泡刺激素(FSH)及雌二醇/睾酮(E2/T)均低于非肥胖组(P<0.05);肥胖组低密度脂蛋白(LDL-C)、总胆固醇(TC)、甘油三酯(TG)高于非肥胖组,而高密度脂蛋白(HDL-C)低于非肥胖组,有显著性差异(P<0.01)。结论(1)肥胖型较非肥胖型PCOS患者存在尤为严重的内分泌代谢紊乱;(2)肥胖型PCOS主要表现为高胰岛素血症及胰岛素抵抗,而非肥胖型则以促黄体生成素升高为主。

Clinical study on the polycystic ovarian syndrome with/without obestty

Objective To investigate in endocringical and metabolic characteristics on polycystic ovary syndrome(PCOS) with/without obesity.Methods 60 patients with PCOS were selected into the study group who subdivided into obesity and non-obesity groups according to body mass index(BMI).Reproductive hormone patterns,glucose and lipid metablic parameters were compared.Results Testoterone(T) was higher in obesity group than in non-obesity,but Luteinizing hormone(LH),LH/ follicle stimulating hormone(FSH) and Estradiol((E_2)/T) were significantly lower than in non-obesity group;low-density lipoproteins(LDL),total cholesterol(TC),triglycerides(TG) were significantly in obesity group higher than in non-obesity group,high density liproprotein chlesterol(HDL) were lower(P<0.01).Conclusion(1) The obese patients with PCOS have more serious imbalances in the endocrinological metabolic aspects than non-obesity ones, E_2/T lower.(2) The obese patients with PCOS show high levels of insulin and insulin resistance,while those of the non-obesity patients have mainly high level of LH.
