空心加压螺钉固定治疗新鲜无移位舟骨骨折的效果评价

目的：探讨应用空心加压螺钉治疗无移位新鲜舟骨骨折的疗效，阐明掌侧微型切口治疗新鲜无移位舟骨骨折的优点。方法：14例新鲜腕舟骨骨折采用AO　3.0 mm无头空心加压螺钉内固定。患者平均年龄32岁，骨折按Krimmer分型，A1型6例，A2型8例；均采用掌侧微型切口入路内固定治疗。术后定期复查X线片，观察骨折愈合情况。以Krimmer评分标准评价腕关节恢复情况及功能状态。结果：术后平均随访12个月，14例舟骨骨折全部愈合；术后腕关节功能优11例，良2例，满意1例。腕关节屈伸活动度为50°~165°，平均（120±25）°。腕尺桡偏为28°~81°，平均(61.0±10.4)°。握力为20~65　kg，平均(50.0±6.7) kg。结论：应用无头空心加压螺钉经掌侧微型切口治疗新鲜无移位舟骨骨折可获得较好疗效。

Evaluation on curative effect of cannulated compression screws in treatment of fresh scaphoid fracture without displacement

Objective To discuss the curative effect of cannulated compression screws in treating the fresh scaphoid fracture without displacement,and to clarify the advantages of palmar miniature incision in treatment of fresh scaphoid fractures without displacement.Methods 14 patients with fresh scaphoid fracture without displacement were treated with AO 3.0 mm cannulated compression screws internal fixation.The average age of the patients was 32 years old.The patients were classified by Krimmer pattern into 2 types: 6 patients were A1 type and the other 8 patients were A2 type.All the patients underwent internal fixation with the palmar miniature incision procedure.The fracture healing situation was observed by postoperation reexamination of X-rays.The wrist joints recovery and functional status were evaluated by Krimmer score standard.Results The average follow-up time was 12 months.14 patients with scaphoid fracture were cured with favorable function,11 got the best function,2 got the better,and 1 got good.The range of motion of the wrist was 50°-165° with an average of(120±25)°.The cubit radial deviation was 28°-81° with an average of(61.0±10.4)°.The grip strength was 20-65 kg with an average of(50.0±6.7) kg.Conclusion Cannulated compression screws by the palmar miniature incision procedure has got a good effect in treatment of fresh scaphoid fracture without displacement.
