血清转铁蛋白检测在病毒性肝炎中的临床意义

摘    要:目的探讨血清转铁蛋白(Transferrin,TF)变化在病毒性肝炎时的临床意义。方法采用免疫比浊法检测100例不同临床类型病毒性肝炎病人的TF,同时检测凝血酶原时间(PT)、血清总胆红素(SB)、血清胆碱酯酶(CHE)。结果各组慢性肝炎(CH)病人血清转铁蛋白与急性肝炎(AH)比较均有显著性差异(P〈0.01),随着慢性肝炎病情加重,血清转铁蛋白逐渐下降,下降程度依次为慢肝(轻度)〈慢肝(重度)〈失代偿性肝硬化〈慢性重型肝炎(CSH)。在慢性重型肝炎和失代偿性肝硬化病人中,死亡组血清转铁蛋白较存活组有显著性下降(P〈0.001)。结论血清转铁蛋白可以作为反映病毒性肝炎病人病情严重程度的一项指标,对失代偿性肝硬化和慢性重型肝炎的预后判断有一定参考价值。

Abstract:Objective To explore the clinical significance of serum changes in patients with hepatitis. Methods Serum transferrin (TF) of 100 patients with different types of hepatitis were determined with immuno turbidimetry and compared with prothrombin time (PT), serum bilirubin (SB) and serum cholinesterase (CHE). Results The serum transferrin of chronic hepatitis groups was significantly difference from those of the acute hepatic groups (P〈0.01). The severer the case of chronic hepatitis damage was, the lower the transferrin was. Enumerated from the highest to the lowest as the slight chronic hepatitis 〈 serious chronic hepatitis 〈 decompensation liver cirrhosis 〈 chronic severe hepatitis. There were negative relationship between serum transferrin and PT. And there were positive relationship between serum transferrin and CHE. In the patients with chronic severe hepatitis and decompensation cirrhosis, serum transferrin in dead group was significantly lower than in survive group with (P〈0.001). Conclusions The serum transferrin was a useful mark to judge the degree of case of hepatitis and prognosis of chronic severe hepatitis and decompensation cirrhosis.
