立体定向治疗高血压脑出血与开颅血肿清除术疗效比较

目的:评价立体定向抽吸术的疗效,探讨其治疗时机及死亡原因。方法:术前CT大片三维定位,局麻下电钻经皮颅骨钻孔,置管于血肿腔,术后尿激酶冲洗,与基本相同条件下开颅血肿清除术疗效比较。结果:治疗组血肿初次清除率在55%,总清除率78%,随访6个月ADLⅠ~Ⅲ级与对照组有显著性差异,死亡率较开颅组显著降低。结论:立体定向抽吸术具有创伤小、手术时间短、疗效确切。在掌握良好适应征基础上具有良好的治疗效果,是一种值得推广的术式。

Contrast of Effectiveness for Stereotactictie and Craniotomy for Hypertensive Intracerebral Hemorrhage

Objective: To evaluate the effect of stereotactic aspiration for hypertensive intracerebral hemorrhage. Methods: The effect of the hypertensive intracerebral hemorrhage treated with CT-assisted stereotactic operation was compared to general craniotomy. Results: Hemorrhage evacution rate for stereotactic group was 55%. ADLⅠ～Ⅲ of patients after 6 months in stereotactic group was singnificantly improved than in control group and the former mortality was reduced. Conclusion: Stereotactic aspiration has advantage of small trauma,short process and credible effect. It is good for applicable technology.
