~(131)Ⅰ对分化型甲状腺癌的治疗(附40例报告)

目的 观察~(131)Ⅰ对分化型甲状腺癌的治疗效果.方法 40例中晚期分化型甲状腺癌在甲状腺全切、颈部转移灶清扫或切除后作了~(131)Ⅰ内放疗.~(131)Ⅰ示33例有肿瘤残留或远处转移,颈、肺、骨、纵隔和其他分别为20、11、8、9、8处.用药1～15次,每次3.76GBq或7.46GBq(100mCi或200mCi).结果 随访2个月～7年,平均3年.病灶消除20处,好转22处,无效5处.临床治愈28例,好转8例,恶化1例,死亡3例,疗效满意.结论 结合治疗结果讨论~(131)Ⅰ治疗的适应证、影响疗效的因素、注意事项及甲状腺全切的意义和实施,强调了~(131)Ⅰ对分化型甲状腺癌的治疗作用.

THE USE OF ~(131)I IN THE POSTOPERATIVE TREATMENT OF WELL-DIFFERENIATED THYROID CARCINOMA:A REPORT OF 40 CASES

Objective :To evaluate the effect of 131I in the postoperative treatment of well-differentiated thyroid carcinoma. Methods: Forty cases with well-differentiated advanced thyroid carcinoma were treated with radioactive 131I after total thyroidectomy and resection of cervical metastatic mass of lymph nodes. After the first dosage of 131l 33 cases revealed remains or metastatic mass in neck (20cases), lungs (11cases), bone (8cases),mediastinum(9cases),and others (8cases) .The patients were given I3II 3.76GBq or7.46GBq (100mCi or 200mCi)for one course and 1 - 15 times accordingly. Results: A follow-up of 3 months to 7 years showed 28 patients were cured while 8 improved, 1 worsened and 3 dead in clinical and 20 masses were disappeared while 22 reduced and 5 unchanged by 131l Conclusion : It was pointed out that 131l is helpful in the treatment of differentiated thyroid carcinoma. The indications and factors affecting therapeutic effect of 131l and management and significance of total thyroidectomy were also discussed.
