颅内血肿微创清除术配合中药治疗高血压性脑出血临床观察

目的:研究醒脑开窍活血汤剂治疗高血压性脑出血的临床效果。方法:将116例出血量≥30mL的高血压性脑出血患者随机分为治疗组60例和对照组56例。两组均在常规治疗基础上进行早期颅内血肿显微镜下微创清除术。治疗组加大黄粉每次1g,每日3次口服或鼻饲;醒脑开窍活血汤剂治疗。结果:治疗组日常生活能力显著优于对照组(P<0.05);治疗组平均催醒时间(28.6h)和再出血率(5.0%)与对照组(45.3h、16.1%)相比,差异均具有显著性(P<0.01)。结论:醒脑开窍活血汤剂治疗高血压性脑出血能明显提高患者的日常生活能力,缩短昏迷时间,减少再出血率。

Clinical Observation on Methods of Restoring Consciousness,Opening the Orifice and Activating Blood Circulation for Treatment of Hypertensive Cerebral Hemorrhage

Objective: To observe the clinically therapeutic effects of agents for restoring conscious-ness, opening the orifice and activating blood circul-ation on hypertensive cerebral hemorrhage(HCH).Methods:116 cases with HCH(bleeding volume≥30mL) were randomly divided into treatment group(60 cases) and control group(56 cases).Both groups were early treated by microinvasive hematoma remo-val operation of intracranial hematoma on the basis of routine treatment. Additionally, containing 1g of rubarb powder dissolved in water was admininstered orally or via a gastric tube 3 times a day in the tre+atment group.Results:In the treatment group,the ab-ility of dailylife(ADL) was better(P<0.05),the average analeptic time was shorter(28.6 hours vs.45.3 hours) and the rehemorrhagic rate(5.0% vs.16.1%) were lower (both P<0.01) compared to the control group. Conclusion:The treatment with methods of restoring consciousness, opening the orifice and acti-vating blood circulation for hypertensive cerebral he-morrhage can increase significantly,improve the ADL,shorten the comatose time, and decrease the rehe-morrhagic rate.
