针刺加静脉滴注川芎嗪治疗椎-基底动脉供血不足46例临床观察

目的运用经颅多普勒超声检查(TCD)观察针刺加药物治疗椎—基底动脉供血不足(VBI)的疗效。方法86例VBI患者经用TCD检测分出低流速型(56例)、高流速型(30例),随机分成川芎嗪组(26例、14例)和针刺加川芎嗪组(30例、16例),治疗10天为1个疗程,运用TCD观察两种疗法治疗前后椎动脉—基底动脉收缩期血流速度(Vs)的变化及疗效,并进行组间比较。结果针刺加川芎嗪组治疗低流速型和高流速型VBI的总有效率分别为93.3%、88.5%,治疗后椎—基底动脉的Vs低流速型明显增高,高流速型明显降低,差异具有显著性意义(P<0.05或P<0.01)。结论针刺加川芎嗪组疗效优于川芎嗪组,并可以双向调节椎—基底动脉血流。

Clinical Observation on Acupuncture Combined with Intravenous Drip of Ligustrazine for 46 Cases of Vertebrobasilar Insufficiency

Objective To study the therapeutic effect of acupuncture combined with Ligustrazine for vertebrobasilar insufficiency (VBI) by transcranial doppler (TCD). Methods Totally 86 VBI patients including 56 cases of low-velocity flow type (LVFT) and 30 cases of high-velocity flow type (HVFT) were randomly divided into Ligustrazine group of 26 LVFT and 14 HVFT patients and acupuncture with Ligustrazine group of 30 LVFT and 16 HVFT patients. After 10 days' treatment, the blood flow rate (BFR) of vertebral artery (VA) and basilar artery (BA) in systolic phase and therapeutic effect were observed by TCD. Results In acupuncture with Ligustrazine group, the total effective rates of LVFT and HVFT were 93.3% and 88.5% respectively; BFR of VA and BA increased in LVFT patients, while decreased in HVFT patients significantly after treatment (P<0.05 or P<0.01). Conclusion As compared with application of Ligustrazine merely, the combination of acupuncture has better therapeutic effect and dual-directional regulation on blood flow of VA and BA.
